As you make your way up out of the vile swamp and make your way back to town, you discover that things have taken a turn for the worse while you were in the swamp. The countryside is dark and burning. Dead bodies line the road back to town. You are forced to fight your way back into the town similar to fighting your way throught he siege in D2. Once back in the town, you see most of the town, except for those guarding the walls huddled around the knight marshall. He instructs all the non warriors to take shelter in the church and that he and the guards and volunteers will defend the town.
He would then ask where you would like to serve in the defense--West wall-safest, east wall- medium, main gate- most dangerous. Depending on where you serve you would encounter different beasts-West wall lower level and weaker flyers and archers, east wall magic users and leaping enemies, main gate burrowing enemies and large brute enemies. Each wall would have a kind of captain that you would eventually defeat. After you defeat the captain, you return to the center of town to see how the rest of the town faired, while talking with the knight, a scream erupts from inside the church.
As you rush inside, you encounter a blood curling scene of corpse and blood. At the altar you see the local preist get torn apart. The girl, or what remains of the girl has turned into a creature similar to andariel. As you and the knight and his men charge, the girl shoots out tendrils which possess the 4 soldiers they suddenly scream as they are devoured by their own armor which fuses together into 4 giant walking suits of armor. You and the knight now have to fight her and the giants.

A place called the Blood Pit, a hole of destruction that can only be found by using very complex magic, as the entrance is hidden. The actual mouth of the hole is invisible. It is covered by 6 feet of floating quick sand that sucks in anything that walks on top of it. Around the actual mouth are 200 feet high cliffs. No person has ever gone down these cliffs without falling off the slippery, bloody, and easily breakable. Then the surrounding these cliffs are what most people call the Black Geysers, highly volcanic geysers that spawn Ash demons and Lava monsters. But if you somehow, get passed the Black Geyers, the Hell Cliffs, and the Sand of Doom, then you will find yoursle in a pit of madness, which has 5 levels.
Level 1, The Collossus Spiral: The highest level is actually a large twisting spiral that goes along pits sides. Bodies and blood are spilled around this path and is guarded by a huge collossus that stand on the middle of the pit from the bottom. He will try to kill you using his hands and mouth, and has claimed the deaths of many who have fallen off the cliffs, into the quicksand, and dropped on the spiral.
Level 2 , The Volcanic Center: As thw spiral comes to a close, you will find your self in the heart of the Pits volacnic activity, which is sewered by skeletons and skulls. The pit then spreads itself in spider-like paths, each leading into lava chambers that spawn the Ash Demons and Lava Monsters. To actually get passed the area, you need to shut off all the Lava Chambers to allow you to go down the tunnels that the Lava takes to get here.
Level 3, Magma Maze: This level involves taking the agonizingly hot and monster infested tunnels that colled off since you turned them off. Your main obstacle is trying to find a way to the actual core without bumping into the dead ends like still heated magma, blocked tunnels, and really magma falls (waterfalls but instead it's magma).
Level 4, The Core: The center and deepest part of this pit is the place called the core. It is a huge lake full of blood, which is a collection of all of the evil blood in this world. At that moment, it was as big as an ocean. This level is mainly guarded by blood zombies that rise form the lake and try to kill you. In the center of the lake, there is an island which takes you to the final part of this madness.
Level 5, The Collossus Den: This is the boss and owner of all of ths pit. It is the home to a huge monster that is known as the Blood Collossus. The one that guards the spiral was his son. This monster is disturbingly huge, and is extremely hard to kill. He has 8 arms: 2 carry shields, 2 carrly swords, and 4 have a bow and arrow. It's think skin has ridiculous amount of defense and it will throw enourmous rocks at you as well.

And I think a cave that is a deformed face coming out of the ground, and when you go into it it swallows you. So You can't get out. And once you're in there it's like being inside a living being, so if you hit the walls in the right place, blood squirts out all over everything. And to get out you have to kill it(many ways to, like taking out vital organs and each has it's own loot, while the heart has the best loot, but is guarded by some champions.). (I just noticed I probably got that idea from Death's signature.)

As much as I agree with you, I am 100% positive that they will have that in Diablo 3. It just seems natural to keep a big factor they had in Diablo 1 and 2.

But that is actually a good idea, but only if it happens rarely, since then it will become too annoying.
My sugestion is not for the Hell but another act of the game:
You are on a ice act (like D2's Act V) and things are silent. The snow are almost grey because the weather is stormy and almost no sunlinght reachs the ground. Then you must cross a frozen lake.
As you walk above the frozen water, small shadows will move under it in your direction. When those small shadows reach you, the thing that was casting it will come out, and it's a demon that looks like one of those weird abyssal fishs. After kill those minor enemies a huge, almost the size of the screen, shadow will pass very fast under you and you will think ''wtf is that ?!?!?!'' and then a huge abobination comes out, looking like a blind giant fish.
This boss will attack you from distance using his tongue that looks like a spiked tantacle/scorpion tail. The tongue only moves in a straight line but when you evade the first attack, another tongue comes out of the first tongue and try to get you. This process reapeats until 3 tongues comes out and then he get that wierd stuff inside he's mouth once again. If one tongue strikes you directaly you will take alot of damage, but if a side spike get you you will loose just a small amount of HP. The tongue are also his weakpoint and the only part of his body that will recieve damage.
He's also summon those small minions, so you must kill then fast before he attacks you, or else you must evade a attack and a minion at the same time, wich is complicated.
He will also spill a black oil in the ground, so if you step on it you will recieve disease damage and your movement speed will decrease, making you more vulnerable to the fishs and the tongue attack. The oil will take a long time to fade, so you must kill this boss fast or else all the ground will be stained.
He's last attack is a strange eclipse and he use it when he gets 40% HP. He's eye start to shine and then the clouds in the sky become so dense that intercept all the sun light. However hes eye keep shinning and the minion fish will also shine in a wierd green color (and you will see those wierd green light swing under the ice). This darkness leave things dificult because it's VERY hard to see the tongue attack. Also lightning will start to falling down in random locations. To survive against this you must hit the giant glimmering eyes, so the darkness dissipates for a brief period.
When you kill this boss, he's tongue will drop out and he will start alot of blood, bodys and stuff, including your loot. Then he becomes dry and grey like a skeleton and fall under the deep water once again.

I would like to have a level called the flesh factory. The boss battle would take place in a room full of pits, there would be chutes that corpses (humans and demons) spill out of into the pits, a giant floating magic drill would come down every so often to mash the corpses into a gory stew that would bubble up from the pit. With it would be health orbs. The boss himself would be a shovel armed freak with body of a hellish centipede, on its back would be mounted fire ballistas that smaller demons would be firing at you. After the boss dies the remaining centipede sections (each having 4 legs) would split up and surround you. If you destroy the center of the boss he splits into two. The bosses main attack would be to try and shovel you into the pits, the secondary attack would be a lightning charged attack, that would emanate from his entire body and shock anything in the direct vicinity (the lightning would arc across the metal studs that hold his skin together), third attack would be to smash you with his shovels. His skin would be a mesh of stitched together husks from various demons (thus the flesh factory).His head... Griswold(his head would be tiny in comparison to his body)
I like the entire factory idea, but I would like to change and add some stuff to it:

1. Monsters are zombies that are deformed and put together by the flesh parts of the factories. They are sewed together and when they are killed, the parts still walk around untill they are literally destroyed.
2. There are hooks on the walls where bodies hang to be worked on (like the butcher's room in Diablo 1) and drills all around you that put together zombies.
3. There need to factory workers everywhere, which are effed up zombies that have have hammers on them.
Changes:
1. The boss is called "The Manager". He is a huge fat monster that carries a huge hammer that does massive damage when hit. His attacks include: Earthquake (slam the hammer on the ground and do damage), Bash (hit you with hammer when you come to close), and Frenzy (will go crazy with the hammer and swings it randomly multiple times. You can't really see his face, since it's all deformed and dead. But he has a huge bloody coat that has worms and guts still sticking out from all the stitches. When he dies, he explodes and all the guts get destroyed and spilled on the ground.
2. There aren't pits all over. But there are boxes everywhere that have parts of human flesh that the factory don't need.
All in all, your flesh factory idea was the best. I just added a couple of stuff.

It would be sweet if the squirrels charged you and did the corpse explode like the dolls in D2. All the monsters in the secret level would be all high level with sick amounts of hit points for good measure make them all extra fast.
The boss of this area would be a big ass unicorn that flies from time to time and rains down acid/poison on you. He would also deal arcane bolts that upon hit do area of effect damage(kind of like FoH). Or he could charge with his horn dealing physical and lightning damage as well as stun your character. All the while he would be summoning more squirrels and rabbits to come chew your fucking nuts off. I mean seriously, as gay as it is these monsters would definitely make you think differently about cute and cuddly creatures. This place would not be for the faint of heart or the weak characters. Oh and another thing. Make it an experienceless level that gives a decent enough drop to consider doing more than once.

And if the drops were staus symbols more than anything useful it'd be cool. a charm that increases light radius and spreads joy an happiness to nearby friendly players. or a book of rainbow blast rank 1.
