I'd like to see unlockable areas open up on the maps you revisit for Nightmare and Hell with side quests to accompany them along with a random quest generator to allow replayability.
So here is how it is works:
1. You just finished normal mode and advanced into Nightmare.
2. Upon loading your first area the random quest generator picks from a list of 25-50ish that has a new area accompanied with it - But chooses 6 of them along with the quests that you have played in normal mode.
3.The new quests will either lead you off the normal map or may unlock a dungeon or an area that was unaccessible before in Normal\Nightmare\Hell.

This could very well happen if two criteria are met:
1. The quests are well-designed quests in the spirit of Diablo 1 and 2, with their own point, plot, dialogue and unique goal, aka not a hodge-podge of quest variables whipped together by the game engine.
2. Regardless of what quests players are given, they are not blocked from beneficial rewards that are only accessible to players lucky enough to score the right random quests. For example, making a random quest liket his given an additional skill point is bad, because some characters will end up with more skill points than others.
If that can be worked out however, then providing a degree of randomness to each playthrough. It worked in D1 as far as I understand, so perhaps this would be a good thing to incorporate.
PlugY for Diablo II allows you to reset skills and stats, transfer items between characters in singleplayer, obtain all ladder runewords and do all Uberquests while offline. It is the only way to do all of the above. Please use it.

First off, I think expecting the game to be different in each difficulty is basically asking for the game just to be a longer game. There was an interview with Jay where someone asked about varying the game in each difficulty and Jay basically said difficulties are just that, harder difficulties, not a new game for each difficulty. However he also said that with the new "adventure system", it would be easy for them to add adventures that only show up in the harder difficulties. To me this is plenty of change to the game for nightmare and hell. I can't honestly expect the game to be different and new after beating it on normal.
