I´ve expericend a very weird bug play solo and with my friend today.
I was playing my marauder chakram in the new season in a T8 rift, them we both died for no apparent reason. in the chat it displayed:
LeonardFS was Slain by a Sentry.
raz was Slain by a Sentry.
Wait what? my own sentry killed me. them we proceed in the rift and the same bug happend, no mobs close, no source of incoming damage. i walked near my sentry and got hitted for 5,5 million damage.
this bug happend with spitfire turret, polar station and guardian turret.
In a real RPG's difficulty settings you have an option to apply real D&D rules, where all damage may affect everything, and you have to take extra precautions not to hurt yourself or friends or npcs especially while using AoE attacks/spells.
Now wouldn't it be cool to have such an option in D3? Sadly all is about memorizing the patterns, optimizing stuff accordingly and rest depends on your brain's reaction latency against what bs game RNG throws at you..

It's possible that you were facing enemies that can reflect projectiles, for example chakrams, impale, rapid fire, ect.
Here's a few monsters that can do it:

when they spin they can reflect physical projectiles back at team mates and kill them
http://diablo.wikia.com/wiki/Sand_Dweller
they have a sand shield attack that on hit can reflect projectiles back at you
