Hidden hand placed (always there) interactive sneaky odd rocks, dusty bookshelves , bloody planks, chains, dusty piles etc. that have no real stand out from environment yet they would be a chance to be movable and a chance to be a dud, if movable there is a chance to drop a upgrade or maybe runes/ gems/gear. These do not light up when hoving your mouse over them, and can only be noticed once clicked and moved. Can drop all tiers of loot.
This effect would make the environment feel more interactive and alive, make you pay attention to environment and want to explore the details.
They kind of had this in D2. For example the beds in the abandoned houses would drop loot if you click on them, but they didn't highlight or have a name, you just clicked them to move and pop, loot.

yeah, i didnt see it in the demo so i wanted to mention it, I really enjoy finding "secrets" in general in games, adds to the mystery
Your eyes kind of learn shapes and colors where you might find secrets, but I agree. Secrets should stay as such until you actually click, not hover.
Well hidden secrets you might maybe click several times at? For example a chain, or a lock, which blends well with the environment.
