I talked to Adria, found the mushroom, but I must have killed the demon and missed it's brain. I can't even remember which of the lava levels the drain should have appeared in. I have just finished the first level of Hell, so I'm certain the brain has been left somewhere in the previous levels. With all my past characters, the brain always appeared while I was in one of the four lava levels.
Can someone please tell me which of the lava levels - one, two, three or four - usually contains the brain, and which type of demon it drops from? I always thought it came from one of the Toad Demon/Overlord or those horned demons that charge at you, however I have not seen any of them in the lava levels.
It depends on where the first demon was when you encountered it. It's not a specific kind of demon; just the first one you encounter after you return to the Caves (the lava levels).
The mushroom patch is on level 9 (the second lava level).
What I do, if I get the quest, is clear level 9 of everything except for one demon before I TP to town with the mushroom; that way I don't have to travel far after I return to the level in order to collect a brain, and I won't have to wonder from which demon it came.
I don't get so lucky as to leave one demon standing every time, so occasionally I'll have to get the brain from the first demon I meet on level 10.
So, in your instance here, I'd search level 9 for the brain, and level 10 in the vicinity of the entrance if you went that far before killing a demon.

If you did that it should be the first one you killed, But I not entirely sure. Never happened to me like that before. One time I cleared out all of level 9 before turning in the mushroom. Had to kill someone on level 10 for the brain.
Alternatively, if you have the Hellfire expansion installed, you could make sure the stairs to town on level 9 are open then go to new game and hope you got the quest again. Without the expansion you would have to ru nthe gauntlet so to speak, all the way to level 9 from level 1.
