My suggestion for D4 would be a sort of "true hardcore" mode, for the many people longing for the feeling that the world is a dangerous and hostile place, where you are very small and not a god like in Diablo 3. This optional mode would have features making the game feel more this way, such as a smaller light ring around the player, no white damage flashes when you hit monsters etc. Additionally maybe reduced chance of finding powerful items, and obviously perma-death. Whatever the devs come up with, for this sort of immersive mode.
Imagine the mode in several horror games, where key items are not highlighted, some helpful HUD elements or tools are taken away, and you truly are afraid to open a door to a new room, if immersed in the game. A mode like that in D4 would be super cool in my opinion.
This would address the demand for higher difficulty (true difficulty, not artificial difficulty with adjusting monster health and damage), smaller power level and the more "grim" vibe if you will, but in a separate mode, to not take away from the large group of fans who like the combat of Diablo 3, and want to (at some point) be able to feel powerful.
