    You can rank people based on the items they possess, but what prevents them from buying those items for real money?

    if the player buys the items for real money they could be bumped up into the next tier but have a very low ranking as they have accumulated little to no exp. to those that do this they probably won't care about their ranking anyway, these people will most likely be PvP players buying items for real money rather than finding/making it themselves or relying on the diablo economy to repair high level items. This is where a seperate PvP "King of the Hill" concept is a good idea seperate KOH points system winning and losing points for kills and losses to elevate you up a seperate ladder of prestige.

    I still think a PvE grading system is a good idea to protect the less experienced players of the game giving them something to strive for and to cut down on rushing of new characters, which I personally don't like, but that's just my opinion.

    I believe a seasonal ongoing graded league of ladders would increase players longevity of playability in game always striving to be the best, instead of first over the line being locked in as No.1 for the next 3 years

    Just an idea, what if the player ranking system was account based instead of being character based?

    You could have an anual "season" with a ladder reset at the end of each year, ranking to be calculated by all of the players in your account their exp and the items they possess. Maybe also points given to players killing other high ranked players PvP an ever changing ladder where players compete for rank for the whole year. The league could consist of three tiers or "grades" eg. A,B,C grade. C grade for noobs, B grade for intermediate players and A grade for diehard Diablo fans. Noob and intermediate players could strive to compete at the top of their grade without being steamrolled by the juggernaught of the higher level diehard diablo players giving high level "hand me down" items to newly created characters. A system such as this could also be used to stem rushing to some extent.
