Further clarification for "Ravings of a Madman": The random spawn area is called "House of Madness" and I found it in Westmarch Heights. It may also spawn in Westmarch Commons as I've seen some doors/dungeons show up in both.
I miss the labels too, but not so much for how much I'm picking up (although it is pleasant to see that "wow!" pile) but so that I know that I got it all. Since the removal of the labels I feel like I'm always missing out on picking up gold, especially on my characters that don't have a big radius.
When starting a new character, I run on expert. I tried doing master for a while and at the start it's not bad at all, tons of levels fast and the trash monsters aren't too horrible; but face an elite/blue/purple monster and it just takes waaaayyyy too long (or is way too dangerous on HC). Expert gives me fast leveling, fast trash killing and decent challenge with the special monsters.
My advice? Open chests and break objects, as well as killing mobs. I've got a lot of legendaries from objects as well as monsters. Don't spend too much time on it, but bring some splash damage or damaging mobility skills and break some barrels, and take the extra seconds to click on a chest.
... and loot the dead bodies. Quite a few of my legendaries both HC and SC have come from barrels and bodies. It seems it really is worthwhile to click everything now.
You missed a spawn location for the Pool; you need to look up at the top too. Just like the other two spots sometimes it's a shrine, sometimes a health pool, sometimes nothing, but sometimes it's a reflecting pool.
I had a Manticore drop tonight (from a dead adventurer outside of Leoric's Manor!).

My DH is only a plvl 1 and hasn't started act 2 inferno yet, and I don't really play her much. So looking at the AH I could probably get around 200 million, or $50 RMAH (US server).
Do these prices seem right?
Did I just hit the jackpot? (I've never had more than 10 mil and I spent that quickly on budget upgrades for my monk)
Is this the primo weapon for DH? Should I sell it for the 200mil or would I be better off just keeping for whenever I do decide to play the DH again? I've found plenty of legendaries before tonight, just never one that seems to have so much potential.
I ran for a very long time with Seven-Sided Strike - Sudden Assault. It provides another ability to get out of bad stuff and can lash out some great damage when used on one or two targets.
Lately though, I switched to Exploding Palm - The Flesh is Weak. The damage that the explosion does really shines as you start ramping up the MP levels (since it's based on their max health). For elites, I'll focus on one of them putting up EP on it. I'll let TC and SW aoe the others and many times when I kill the one I'm focusing the others are very close to dead if not outright killed from the EP explosion.
... what are they afraid of? that he mite survive Inferno.....oh no! can't have that lets weaken him some more.
Do you have access to a beta version the rest of us do not? How can you possibly know what is working on anything past SK, let alone on a different difficulty?
Obviously, they felt it needed some tweaking based on their numbers from internal testing where they have many people playing at the higher difficulties, not from beta testers who can level all the way to 13 and in order to do that must repeat the same content over and over and over again.
All I am saying is give the Barb a break, soon there will not be any decent skill to go with because he will be too balanced and no skill will be efficient enough in tougher area's......
Too balanced?! I don't think it's possible for anything to be too balanced, they either are balanced or not balanced, no such thing as too balanced. But that aside, what are you using as the terms of your "balance"? Barb skills vs. Barb skills? Are you looking for one particular skill that does so much damage that we all feel compelled to use it? I hope not. Let them continue to tweak, I say. If they achieve their goals, we'll all use the skills that we find the most fun and tactically efficient for the situation.
Sometimes I wonder why Blizz even releases info like this. Perhaps Blizz enjoys watching people go all wacko with no rationale for their rants. Beta patch notes == Troll?

To make the game harder, you don't need to change out all your gear, you only need to press one button!
Push 'z' and commence to trying to kill stuff/find your way around.

It was in Westmarch Commons, IIRC, however it drops after a random event (involving the madman) in one of those random spawn doors.

The timer is a backup system in case you get a bad RNG streak. It will gradually push RNG in your favor. So yes, upping T levels does matter.
