It's quite possible that the Three didn't rule Hell in Diablo 2, but still they weren't trapped in the mortal realm by the Soulstones since Diablo was able to return there with his still stuck on his forehead.
They simply fooled the lesser evil in kicking them out to trick the Angels and give them a reason to break the pact Mephisto made with the Council to stay away from the mortal world (since they don't control Hell anymore the demons will now be able to invade Sanctuary).
At the end of Baal's run, he dies and purges a great many spirits from his body, all of which swirl around for a moment before heading straight into the World Stone. Now, I didn't notice this before but these spirits can also be seen in the cinematic, "Destruction's End", where they are mere blurs in the background as Tyrael mumbles in some unfathomable tongue.
Yep, and when you destroy Mephisto's Soulstone, a lot of spirits are released from it as well and it was probably the case with Diablo's too.
You know what I think? I just rewatched the Terminator trilogy not so long ago and I believe that the Prime Evils' souls are just like the liquid terminator. Regardless of how much you fragment them it will always be able to rebuild itself, and that's how I think the Three will return. It seems that the destruction of the Soulstones served anything but to delay their return.

Tyrael isn't belial. In Legacy of blood Tyrael stopped the rise of the warlord of blood. The warlord of blood is a consort of belial. That was in a time period while Diablo was in Aaranoch during Diablo 2(Lut Gholein). You first meet Tyrael in Aaranoch.
QTF, but I think that it could be possible that a Fake Tyrael who is actually Belial in disguise returns in D3.

I think the prime evils wanted their soulstones destroyed so that they could return to hell and reclaim their throne. Before they returned to hell they needed to soften it up.
Reclaim their throne? No offense but did you even play to D2? At the end of Act3, Diablo went in hell to announce the return of the Three and rally an endless army of demons to invade Sanctuary. Where do you think that all those demons came from in Act 5, there are even portals that lead to Hell in the different zones of the Act.
I don't believe that they wanted their Soulstones destroyed. I think they wanted to use the Worldstone's powers to enslave mankind and acquire the true power of the Nephalem so they could win the Great Conflict and destroy all creation.
The destruction of the Worldstone only delay the inevitable as a new generation of Nephalems is coming and you can be sure that the Three will find a way to possessed the strongest amongst them, one way or the other.
I just had a crazy idea. Following to Uldyssian the Worldstone is "in truth everywhere" (book 3 p.305). By spreading their essence into the Soulstones the Three did not only became able to manifest themselves into the world, but they actually spread their essence everywhere in Sanctuary. Thus even if the Soulstones and the Worldstone were destroyed it may indeed be still possible for the Prime Evils to come back to the mortal world.
As for after the Sin War trilogy, we can only assume that something started happening to people all over the world after the death of Uldyssian.
There's no official information about it, but I have a feeling that the Horadrim might be some sort of third generation of Nephalem, notice how close it sounds to Edyrem.
If only lessening the power of the Worldstone over humanity created a godlike mageclan we can only imagine what its destruction will do over time.
Well, no one really knows but considering that they can live for a very long time, it is very likely that some of them are still alive in a way or another like the Guardians of the Arreat Summit.

And why has the power of humanity diluted with each passing generation? Or is this the effect of the World Stone's neutralizing powers?
Well if you really want to know, Inarius hated the Nephalem because he thought they were to powerful and hazardous, so he used the Worldstone to turn there offsprings into powerless little mortals. The Worldstone doesn't necessarily have neutralizing powers, in fact Uldyssian used it to reverse Inarius spell which in turn most likely created the heroes we know nowadays.
Are the Void, the Abyss, and the Burning Hells all different?
I think the void actually refers to the domain of Trag'Oul whereas the Abyss probably refers to a particular region in the deepest reaches of Hell.

Now, Inarus was the one who gave the three the information on how to corrupt the soul stones, but who says he told them the truth?
Dude, it wasn't Inarius, it was Izual.
Warning, the rest of this post contains spoilers from the Sin War Trilogy, you've been warned
There's an important point that all you guys seem to have forgotten. The Worldstone was used by Inarius to neutralized the powers of the Nephalem, and with its destruction the Nephalem will inevitably return. If Tyrael is indeed dead or corrupted, it is very likely that with the influence of the rest of the Angiris Council that Heaven will turn against humanity. It won't be Sanctuary + Heaven Vs Hell anymore, it will become a three sided war with Sanctuary at the middle.
Also I think that it's possible that the destruction of the Worldstone was planned by the Three from the beginning. Notice how much Fara or Cain put the emphasis on how much Tal Rasha's powers are making Baal stronger. Now imagine just how powerful a Demon like Diablo would be if he possessed a Nephalem like Uldyssian. He would literally become an invicible God of Terror and crush both Sanctuary and Heaven with a sneeze.
That's what I think the Three were after for all this time, and the question now is how are they going to do it?
