Izual never destroyed Shadowfang, which was what he was trying to do. When Tyrael give you the Izual quest he says Izual still poses a challenge even though he does not have the Azurewrath anymore.
Both Shadowfang and Azurewrath can be found in the game. It is possible, though very rarely does it happen, that Izual will drop Azurewrath (which is kinda funny coz Tyrael said he isn't suppose to have it anymore)
Shadowfang is really only mentioned the one time, it is not clear who wield it after the failed attack on the Hellforge, if indeed anyone ever did. The only monster who probably had any real contact with Shadowfang is Haephastos the Armorer, as he probably made it, being that he works the Hellforge.

D1 and D2 Goat men are in Hell, then they come to the mortal Realm. Sure it says believed, but it also says in Hell, meaning they at some point have to be in Hell.
According to ancient carvings that I have succeeded in translating, the khazra were originally human, part of the umbaru race found in the thick Torajan jungles in the Teganze region of the eastern continent.
D3 goat men were clerly human at one point.
I find it hard to see how one set of goatmen who have always been goatmen and were once in Hell then came to sanctuary are the same as another set of goatmen who were human at one point living on sanctuary and were later made into goatmen, and never once set foot in Hell.
I do not deny that Baal could have had a hand in making the d3 goatmen, I just find it almost impossible for them to be the same ones from d1/d2, based on what is written about them. And from what I have seen there is little to no contradicting going on here.

D1 and D2 goats are the same. It says they are Baal minions and come from Hell.
D3 goats are said to be members of the Umbaru clan who were mutated in experiments. That is the clan the Witch Doctor is from.
Kinda hard to see the all as the same goats.

The evil did attack others. It just started with Tristram because that is where the Cathedral was. The Cathedral that stands over the place where Diablo's soulstone rested for 2 centuries.
Also other evil attacked the other villages as well, Ogden says some black riders had been attack villages nearby Tristram.

As said by myself, and several others, present in Tristram are various military out posts build by king Leoric. Also there are several other villages which are mentioned by Ogden in d1, and some other NPCs.
There is also a castle somewhere too.

I agree, a (almost) complete, detailed, concise, and exact time line of all the events from the beginning, right up to the start of D3. This sort of thing is very interesting to me, and I am sure lots of others too.
If there was a petition to get a time line I would sign it, without question. Screw complaining about the art (which companies seldom listen to it seeems) I want to know what year Leoric came to Tristram in. What year did Izual assault the Hellforge, how long was it between Leoric coming to Tristram and him going mad.

Xiansai is the name of an island. Few information is given about it prior to D3, not even it's name. It's name was given in D3.

When Richard Knaak wrote the Sin War Trilogy he rewrote large portions of the Diablo lore, and according to Blizzard, what he wrote is more canonical than the games. The games came out first and Mendeln does not appear in the first two games.
He could appear in Diablo III is some way, but to give a short answer, no he does not appear in any larger capacity out outside of the Sin War Trilogy, just like many others mentioned in the book. Such as the servants of the three, Lucion, and pretty much every angel except Tyrael and Inarius.

During Jehren's quest activation speech for the Arcane Sanctuary quest, he makes mention of the fact that Lut Gohlein now stands on what was once a Vizjerei Fortress.
Are there any other speeches or allusions to this. I would very much like to know the name of this fortress. Though that is probably unknown, however due to the fact that the Arcane Sanctuary is there as well it is fair to say the the Fortress is Horazon's, or at least that he was in charge there.

Inarius sent a group of minions to attack a temple dedicated to Mephisto. Mephisto took his revenge on Inarius and his followers. But I think the Sin War Trilogy overwrites this. That was the original reason.
But its still essentially the same. Inarius does something that Mephisto takes extreme issue with, and punishes him.

I would think the Prime Evil with the strongest essence would take control and the other two remain in the soulstones.
While they are brothers (in arms) I doubt either one of them would be willing to share a host with one or both of the others.
