Mods from the Phrozen Keep usually incorporate references to other works, most often in the named of unique and set items. Monsters are also commonly from the Baldur's Gate/Icewind Dale series, since animations from the Infinity Engine apparently lend them selves very well for Diablo file format conversions.
I don't know of a mod that is entirely based on another fictional world though.
PlugY for Diablo II allows you to reset skills and stats, transfer items between characters in singleplayer, obtain all ladder runewords and do all Uberquests while offline. It is the only way to do all of the above. Please use it.

There was also a mod geared towards Baldur's Gate.
Most other mods have used some of the animations from the Baldur's Gate and other games from that genre since like Phrozen has stated the conversion process is not that difficult.

On Strike and supporting Fallout 4 Mod Makers
Some fallout 4 mod makers have had their mods stolen and uploaded and downloaded on Bethesda's site for the Xbox One.
Not entirely based, but with items from others. I would love to play a necromancer that has Artemis Entreri's famous dagger, or an Amazon that uses Taulmaril and Khazid'hea from Catti-brie. Drizzt's famous swords for the assassin, Guenwyvar for the Druid, and Aegis Fang for the Barbarian.
