It's possible but that's just a little too obvious to be the case. I'm sure the lore people thought they were really smart in tricking people with the possibility leah becomes diablo, as they basically give away for free that her mother was a witch and her father was the wanderer who was being overtaken by diablo.
However, I think that while Leah may have some role to play in all of that- she doesn't become diablo. They may very well be linked and diablo may very well use her, as he somehow gets the black soulstone which is in her possession at some point. Leah's mother is more interesting to me than leah is.
Also keep in mind diablo's essense is no longer trapped within a soulstone and the worldstone that once hid Sanctuary, in the form it was in, was destroyed and so existing demons basically have free passage to and from the world.
As for the appearance of Diablo, they say what they want and expect the fans to either believe it or suspect the next obvious thing, "oh he's got nice legs" and suspect leah. Again, could be, but it's even more likely that

We're not exactly positive what it does. But if you look at what soulstones have done in the past (contain prime evils), and look at what azmodan has to say about the black soulstone (it is "the key", and he will use it to become the prime evil, or how the person azmodan is talking to "tricked his bretheren who fell one by one"), then you also consider the datamined info where Diablo is using the essences of other prime/lesser evils, and given the images of diablo where he looks like the other evils+himself merged into one (like the legs of andariel, the arms of mephisto, the face/horns of baal)... Maybe the soulstone is a way to merge entities into one, or trap the essence of weakened entities. Maybe diablo is trying to re-create the ultimate being, or his cut off spawn tathemet, to put an end to the war between heaven and hell. Who really knows. The soulstone may not just stop at merging prime/lesser evils. Originally, demons and angels came from one all powerful being. I don't think it can effect the nephalem, seeing as how they are basically similar to the original being Anu and not a demon/angel, however who really knows.
In the book of cain, there are some details from deckard's ancestor about the hunt for the prime evils. It notes that tyrael was impressed with the sacrifice of Uldyssian who was a Nephalem (demon+angel hybrid) basically on the verge of having god like power beyond that of demons and angels but chose forsake those powers and even his humanity to save his world. That was the point at which Tyrael starts believing in the potential of humans(nephalem) and I do believe then later instructs the horadrim how to use soulstones to trap the prime evils in sanctuary. For more information about Uldyssian you can check out the wiki but I think he realized over time that he and others had the birthright of the nephalem and ultimately made his way to the worldstone and removed the limitations it had on him and his edyrem followers. His power being such as it was (as a nephalem with powers from angels and demons/lillith), what he had done to the worldstone could not be undone by inarius and overcame inarius ultimately. only to be shackled by tyrael. Seeing his bretheren battling both heaven and hell he was eventually able to break the shackles from tyrael and froze time and removed the angels and demons from sanctuary. He tried undoing all the destruction that had occured, but his powers started to disobey him and destroy, he felt he had no other choice than to bottle that destruction within himself and release it in the void. Tyrael was also the deciding vote (after the sacrifice, and despite the fact he originally thought very little of humanity), to allow the nephalem to continue to exist, supposedly outside the influence of demons and angels (which infuriated imperius who thought highly of tyrael beforehand and saught the destruction of sanctuary from the beginning). This however obviously was not to be, and so the conflict between heaven and hell continued affecting sanctuary.
One of the horadrim specifically entrusted with the soulstone by tyrael was named Zoltun Kulle. During their battle with (I think it was) baal, one of the nephalem notes Kulle was mishandling the soulstone, which resulted in it being shattered into smaller pieces during the battle, which meant the stone was no longer powerful enough to contain the evil itself, and ultimately led to Kulle suggesting someone trapping baal and his essense in the stone in themselves- leading to the untimely death/posession of the leader Tal Rasha (IIRC). Later after all was said and done Kulle becomes obsessed with the soulstones. He became so powerful that he could not die, so the horadrim sought him out and apparently cut him into two and sealed his body.. However it has been revealed in a gameplay video even that Kulle is up to no good in the underground vaults of act 2 Diablo 3, and that he's been creating or created a black soulstone which can contain the essences of multiple beings.
This combined with the fact that Leah has the black soulstone somehow (I'm guessing after act 2 leading into act 3) and she forsees azmodan seeking the stone for power in order to become "the" prime evil, and also combined with the fact that in datamined information also posted here in the lore (I do believe) diablo uses the essences of other prime and even lesser evils.
All of this makes sense when you consider all of the evils were once one of the 7 heads of a massive dragon entity Tathemet split off from Anu, the original celestial being who saught to be free from inner turmoil.
So the evils are trying to use humanity, and the angels are somewhat split about whether to destroy humanity to prevent them from being used (or the fact that they're half demon) or let it have a chance and play out. The horadrim have been the evils rivals in terms of power from sanctuary(Uldyssian aside), however they may be what gives the evils their greatest opportunity for greater power with this whole black soulstone ordeal w/ Kulle.
However now with the tampered with, alternate reality stolen worldstone destroyed- even if the prime evils abuse the new highway from hell to sanctuary, and the black soulstone, they still have to deal with nephalem who are, like Uldyssian, finding their powers are growing quickly, not even altered by a world stone.
-I'm fairly positive that the primes (and their minions) can come and go from sanctuary as they please now that the warp-reality worldstone has been shattered and arreat is now supposedly a hotspot of celestial/demonic energy for passage due to that. Which leads me into the question, why does diablo need to possess anyone, unless he is trying to hide himself/trick someone? There should be nothing preventing him from manifesting himself as he truly is.

I won't spoil too much (though some is still uncertain), but the black soulstone is more of a way to generate/steal power than manifest one's self in a hidden world via a possession, seeing as how the world is no longer hard to find/travel to. Azmodan claims he will become the prime evil using the soulstone.
