Second, there's no clear yes or no answer, as it's a bit more differentiated. Note that "proc" is the wrong word to use here; a proc is something else. But anyways, Davlok has the answer:
Madstone with MR: (no Gungdo)
Target without EP: Madstone applies a fresh non-MR EP and detonates it.
Target with EP: Madstone does NOT apply a new EP (refreshes the duration) and instantly detonates that original EP. All subsequent SSS strikes apply a non-MR-EP and detonates immediately as expected. If the original EP was MR-buffed, only that single EP detonation would occur and the rest would be "plain" EPs.
Madstone with MR: (with Gungdo)
Target without EP: Madstone applies a fresh non-MR EP and detonates it. That first EP is then "refreshed" by Gungdo, then "refreshed" by Madstone on the next strike, and then detonated again for the rest of the SSS.
Target with MR-buffed-EP: Madstone does NOT apply a new EP (refreshes the duration) and instantly detonates that original EP. Gungdo then refreshes the MR-buffed-EP on the original target and Madstone keeps refreshing the EP Gungdo just re-applied. So all explosions are MR-buffed, and Madstone is really doing nothing useful in this case.

I think it depends on the group. If the group is slower than sharding is the best way but if your group is smashing the content then full clears are way better. I like doing full clears though just because gob packs and exp. Kadala is a cop out

Is there a guide out there that any of you doc's could link me to that can provide a break down of fetish attack speed break points? just looking for any resources that could help me better gear my pet doc. thanks to anyone willing to help

It's season one bro. Relax. They are trying to get shit right. Are you really going to care two seasons from now that in season one shit changed? Probably not. It's really not that bad. And I think d3 has come a long way. If you don't like it just stop playing until season two comes out.

The way I see it is you're getting a free chance at gear for something you were going to do anyway... Rifts. Who cares what they do with Kadala. You'll be getting most your gear from rifts anyway. Kadala is just a way to target a single leg. Sorry but I feel like everyone complaining was in those rift if foward communities and just didn't even play the game to get their gear. Sorry you actually have to kill monsters to get gear now. Must suck.
