Basically it says that you sum up all the minimum physical dmg stats on your character (from sources/rings/ammys/weapons), multiply it by 2, take the percentage, and add that value as damage.
My question is not what constitutes the % dmg, but how is it delt.
Example:
Naked wizard with 0 intel
200-400 black dmg weapon
a ring with nothing but Adds 15% to fire damage.
If he uses an attack that does %200 weapon dmg will it hit the mob for:
A: 2*300 + .15*(200*2) = 660 damage
or
B: 2*(300+ .15*(200*2)) = 720 damage
?
In Case A the +ele dmg acts like an instant direct damage attack, where as in Case B the +ele dmg acts as an increase to your weapons base damage similar to an elemental damage affix and thus is accounted for in percentage based damage attack calculations.
Which of these 2 cases(or neither) reflects how it works in game?
And to add a further complication is the %dmg calculated with or without taking into account intelligence?

If I'm understanding your question correctly, the math has been tested and proved to match the in game damage number. How they figured that out I have no idea since I tried to calculate it myself a few months ago with a Tal neck. After reading the method, all my tests gave accurate results. I have also tested it in the current patch with the Zunimassa boots and can confirm the calculations are unchanged from previous patches.
EDIT: Also, the tooltip on the items appears to work correctly for relative dps comparison. However, I noticed that the tooltip doesn't seem to take the set bonus into account, but that's a little off topic.

Yes, it is very misleading. It also only applies to physical damage, which makes it even more confusing. If you have +holy damage on your weapon you have to actually subract the min value from your min weapon damage before applying the calculations for the bonus elemental damage, even if the item is +x% holy damage.
