I mostly play multiplayer but have recently started to test my skills in single player. I know follower can be important, so which follower should I use? What skills should I give my follower? And how should the follower be geared?
As a monk, there is no best follower, only a follower that suits your needs at the time.
The Templar gives 12% Spirit Generated as one of final skill choices which is quite useful if you run a Tempest Rush build.
The Scoundrel gives 3% Critical strike as one of the final skill choices, which is most useful for Sweeping Wind: Cyclone as this can up your tornado spawn rate, and just an all round DPS lift.
The Enchantress gives you Force Armor in the second tier which gives you a 5% armor buff or in the final Tier a 3% attack speed skill. The enchantress gives out a lot of crowd control, so if you are struggling to survive, her little bits of control can give you the upperhand.
If you are not Paragon Level 100 yet, it is best to gear your follower with max magic find items, and %exp gain, as you will gain 1/5 of those stats towards your own. Say you get your follower to 100% MF and 35% exp gained, you yourself gain 20% mf and 7% exp gained. Other than those two stats in particular, your follower gets large bonuses from their main stats, eg Vit for all, Dex for Scoundrel, Int for Enchantress and Str for Templar.
3.) The enchantress adds usaly less paper dps to you but in her 3rd skill tier she got erosion. Its just a 3sec dmg buff but if you manange to have an elite in the pool and line up your burst its a decent dps push.
Agree with everything but one. Even if the Scoundrel dies you keep the 3% CC buff.
I personally run Scoundrel multishot with Buriza and Star of Azkaranth on all my characters for the freeze. Even if he doesn't freeze, he always chills.
My only add is that I prefer a follower who engages opposite my char: Monk = melee, so use enchantress or scoundrel that uses a ranged attack. When I played a DH, I used the Templar. The only downside to the ranged attack is they like to take a crack a goblins when you're not ready for them to do so.
I've tried having followers add DPS as I thought a follower with 10-20k DPS would be similar to a 10-15% increase of my own DPS (about 100k back then).
I was dead wrong. They simply don't have the "250% weapon dmg AoE" skills that the heroes have, nor any way of casting multiple skills in sinergy, hence their dps is immensely subpar (compared to heroes). Even if we could get followers to 40-50k DPS I probably still wouldn't use them that way.
I'd say Crowd Control + Survivability is definitely the best way of gearing a follower nowadays. LoH works wonders for the Scoundrel.

I would suggest Life after Kill and LoH. Even on mp10 my scoundrel does not die very often now.

I seriously doubt your follower will get anywhere near enough kills to profit from Life on Kill. LoH on the other hand is very useful, of course.
I believe your kills count too... at least that's the way it seems. I may be wrong (no "science to back it, just my feel)... but when I melt a pack w/ EP, my scoundrel goes from almost nothing to full health the blink of an eye.

That gem alone costs you 120m, you know that?
If you want a DPS follower, go Scoundrel (I buffed mine to ~18k DPS for 1-2m gold).
Maximus is only good for tanking, there are better swords to get high DPS; besides, on high MPs the Maximus' demon is useless as tank.

Yeah, so what^^
I disagree... only playing MP7-8 here for farming, he does what he is supposed to do.
He is taking off the Heat thats ment for me or my Follower
I played a lot with the Scoundrel, and he always died.... ALWAYS.
Cant remember when my Enchantress last died
